    COVID-19 vaccination in Northwest Wisconsin

    Vaccination for ages 5–11

    The Food and Drug Administration has granted emergency use authorization of the Pfizer COVID-19 vaccine for children ages 5–11. Eligible patients in this age range for the Pfizer COVID-19 vaccine have been identified and will be contacted through Patient Online Services or by mail, and be invited to schedule an appointment.

    A parent or legal guardian must make the appointment for children age 5‒11. Children must be accompanied by parents or legal guardians when they come for their vaccination appointment.

    Parents and legal guardians can schedule COVID-19 vaccinations using their children's Patient Online Services caregiver account or the Mayo Clinic app. If you don't have a caregiver account for your child, call Mayo Clinic Customer Assistance at 1-877-858-0398.

    Parents and legal guardians also can call one of the locations below.

    Vaccination for ages 12–15

    Parents or guardians for Mayo Clinic Health System patients who are age 12 can call a location below to schedule an appointment or schedule via a Patient Online Services caregiver account. Patients ages 13 and older can directly schedule their vaccination appointment using Patient Online Services or the Mayo Clinic app. Check availability in the "New Appointment" section. Select the "COVID-19 Vaccination" box to determine if times are available, or call one of these locations to request an appointment:

    Children under the age of 18 will need to be accompanied by a parent or guardian to their appointment to provide consent for the vaccination. Alternatively, the child can bring a signed consent form completed by their parent or guardian. In the “Limitations” box on the form, please write “Pfizer-BioNTech COVID-19 vaccine.”

    COVID-19 booster vaccinations

    Three types of vaccines have been authorized and approved. You may get a booster vaccination if you received:

    • Pfizer
      Are age 12 and older and it's been at least five months after completing the primary series; ages 12–17 may only get a Pfizer booster
      Are age 5–11, it's been at least 28 days after completing the primary series, and you are moderately or severely immunocompromised
    • Moderna
      Are age 18 and older and it's been at least five months after completing the primary series
    • Johnson & Johnson
      Are age 18 and older and it's been at least two months after receiving the Johnson & Johnson vaccination

    Parents and legal guardians can schedule COVID-19 vaccinations using their children's caregiver account in the Mayo Clinic app or Patient Online Services. If you don't have a caregiver account for your child, call Mayo Clinic Customer Assistance at 1-877-858-0398. Check your nearest location above for additional scheduling details.

    Mix and match vaccine strategy

    Current recommendations are that the booster vaccine be the same brand as utilized for the initial vaccine series. However, Mayo Clinic will follow the mix and match strategy approved by the Food and Drug Administration in cases:

    • Where a provider or patient feel a different vaccine brand would be more appropriate
    • Where there are concerns about the side effect profile of a particular brand
    • Where the vaccine brand used for the initial vaccine series is not available

    Scheduling your booster vaccination

    Schedule an appointment online using Patient Online Services or the Mayo Clinic app to receive your booster vaccination. You also can call your local clinic in Northwest Wisconsin to schedule a COVID-19 booster vaccination.


    Reasons to delay your vaccination

    You should delay scheduling your vaccination if:
    1. You are in quarantine from a recent COVID-19 exposure.
    2. You received a bone marrow transplant within the last six months.
    3. You had a recent COVID-19 infection or a positive COVID-19 test. While you may be vaccinated once you are released from isolation, consider waiting 14 days from the COVID-19 diagnosis because you are at lower risk for infection than others.
    4. You received convalescent plasma or monoclonal antibody therapy in the last 90 days.

    Questions about delaying your vaccination should be directed to your care provider.


    Continue to be safe

    While vaccines are effective in preventing symptoms from the COVID-19 infection, you should continue to observe the safety measures that are known to reduce transmission at this time:
    • Wear a mask that covers your nose and mouth.
    • Stay at least 6 feet apart from other people.
    • Wash your hands with soap and water for 20 seconds or use hand sanitizer with at least 70% alcohol.

    • During your appointment

      • Wear a mask.
      • Bring your insurance card.*
      • Please arrive on time. Note: To help maintain proper social distancing, if possible, do not enter the building more than 15 minutes before your scheduled appointment time.
      • If possible, please enter the building by yourself. This helps maintain proper social distancing. If you need assistance, your companion will be welcome.
      • When you arrive, you will be asked to complete the check-in process at a kiosk station. Assistance will be available if needed.
      • Vaccines will be administered in private rooms.
      • Expect your appointment to last 10 to 30 minutes.
      • If the vaccine you receive requires a second dose, be sure to schedule a second appointment before leaving your local vaccination site or by self-scheduling through Patient Online Services.

      *The COVID-19 vaccine will be free. However, vaccine administration charges will be billed to insurance. We anticipate that the fee for vaccine administration will be covered by the payer at 100% and patients will not be billed for any unpaid charges.

    • After your appointment

      • Continue to wear a mask, stay 6 feet apart from other people and wash your hands often.
      • If the vaccine you receive requires a second dose, you will be asked to schedule a second appointment before leaving your local vaccination site.
      • Monitor yourself for possible side effects, such as pain, redness or swelling where the shot was given; fever; fatigue; headache; muscle pain; chills; or joint pain.
        • If you have a reaction, take some time to rest, and allow your body to recover. Stay home if you have a fever.
        • Having reactions to the vaccine does not mean you have COVID-19. It is not necessary to have a COVID-19 test or to quarantine.
        • If you have reactions that prevent you from eating, sleeping or going to work, contact your health care provider. Also contact your health care provider if you have reactions that last longer than three days.
        • Signs of an allergic reaction include hives, swelling of the face and throat, difficulty breathing, a fast heartbeat, dizziness and weakness. If you have any of these signs, call 911 or have someone drive you to the nearest Emergency Department.
