There are three easy ways to refer your patient to Mayo Clinic Health System: Online, by phone, and by fax. Physician Referral Services is your central point of contact for referring patients to any location.

Refer online

The Mayo Clinic CareLink Portal allows you to refer into Mayo Clinic Health System online, without having to call or fax in any forms. With a patient consent on file, you will also have immediate access to your patient’s health records and test results including:

• Clinical notes
• Discharge notes
• Radiology & pathology reports
• Test results
• Surgical & visit summaries

Create a new CareLink account or sign in to CareLink to begin referring patients to Mayo Clinic Health System online.

Refer by fax 

• Minnesota locations
  Contact each Minnesota location directly to refer patients by fax.
• Wisconsin locations
  Fill out our referral form on your computer, and then print and fax it, along with your patient's most recent progress note, to 1-855-392-9335. You can also call 1-855-392-8400 to have our staff confirm all necessary information for the referral and route your request and records to the appropriate department for review.

When the appointment has been scheduled, we will notify you. When the appointment is complete, the department that has evaluated your patient will send you records from that appointment/encounter within three business days.

Follow-up

You will receive a notification from us once your patient's appointment has been scheduled. After the appointment, final notes and any results will be delivered via CareLink in-basket message, or faxed to you within three business days.
